Key Responsibilities\n

• Provide live support to our interdisciplinary Practitioner team to identify immediate risks associated with clients and initiate de-escalation strategies as needed.

• Take the lead on escalating situations that require emergency services, ensuring that mandatory reporting procedures are followed when a client’s identity is known and reporting thresholds are met.

• Regularly inform Clinical Leads of newly identified safeguarding risks, actively contributing to the development and implementation of mitigation strategies.
\n
• Stay current with all safeguarding policies and procedures, implementing these standards in day-to-day operations while addressing any challenges to adherence.
\n
• Utilize outcome-focused practices by collecting feedback and insights from clients, identifying barriers to progress and making recommendations for improvements.

Patient Population / Client Focus\n

In this role, you will work primarily with individuals and families facing terminal illnesses, providing psychosocial support to help them navigate the emotional complexities associated with end-of-life care. This includes working with children, adults, and families to foster resilience and coping strategies in challenging times. Your expertise will facilitate crucial conversations about advance care planning and emotional support, ensuring dignity and grace throughout the hospice experience.

Qualifications\n

• Master's Degree in Social Work from an accredited graduate program is required.

• Active Licensed Clinical Social Worker (LCSW) certification is mandatory.
