RN Diabetes Educator II
The RN Diabetes Educator II is responsible for providing quality diabetes education through assessment, education, goal setting, and counseling. Addresses the following topics: diabetes mellitus, pathophysiology, blood glucose monitoring, safe and effective use of diabetes medication, insulin preparation and injection skills, signs and symptoms of hyper- and hypoglycemia, sick-day management, foot care, and follow up care. Provides clinical expertise and training on diabetes management to professionals caring for people with diabetes.

What you will do:
- Assesses each patient referred for Diabetes Self-Management Training (DSMT) or diabetes education, identifying and evaluating the specific problems and needs.
- Provides DSMT and/or diabetes education specific to the needs of the patient, accounting for age and culture specific differences.
- Monitors clinical and behavioral outcomes of patients receiving DSMT and/or diabetes education, revising treatment as needed and when possible.
- Assists the provider with the management of diabetes-related medications.
- Refers patients to other team members or community resources as appropriate.
- Provides clinical expertise and training on diabetes management to the staff and departments within the hospital and to the broader community.
- Attends hospital committee or program meetings that relate to diabetes care impacting both inpatient and outpatient clinical practice.
- In the outpatient setting, participates in developing and coordinating the clinical and education components of the Diabetes and Nutrition Education Program, consistent with national standards and guidelines of the American Diabetes Association and the American Association of Diabetes Educators.
- Performs all duties and responsibilities in accordance with the C-I-CARE Standards of the hospital. C-I-CARE is the foundation of Stanford Healthcare Tri-Valley's patient-experience and represents a framework for patient-centered interactions.
Education Qualifications:
- Other Graduate of an accredited School of Nursing Required
- Bachelor's Degree Bachelor of Science in Nursing (BSN) Preferred
Experience Qualifications:
- 3+ years to 5 years of experience in a relevant setting, with preference for the experience to be within the last five (5) years Required
Required Knowledge, Skills and Abilities:
- INPATIENT SPECIFIC:
- Assists with the creation/revision of patient education materials or patient classes
- Regular and active participation in a hospital committee that meets at least six (6) times per year; brings back pertinent information to the department
- Performs key role in at least one performance improvement initiative or creates/revises diabetes-related protocols or programs
- OUTPATIENT SPECIFIC:
- Assists with the creation/revision of patient education materials or patient classes at the discretion and direction of the department leadership
- Assists in the orientation and training of new employees at the discretion and direction of the department leadership
- Performs a key role in at least one performance improvement initiative annually. This includes but is not limited to:
- Regular member (80% attendance or more) that meets at least once a month or until completion of its assigned task
- Actively participates during meetings and does relevant work outside of meetings
- Brings information back to the department, disseminates this information, and helps with implementation
- Assists in the accreditation and maintenance of department licensure by collecting data, preparing reports, and communicating with other departments
Preferred Knowledge, Skills and Abilities:
- Comprehensive knowledge and application of diabetes management tools.
- Critical thinking to integrate facts, informed opinions, and observations to support decision-making in patient care.
- Effective communication in written, verbal, and electronic form to health professionals, medical staff, patients, and public.
- General knowledge of electronic health records and common computer applications.
- Strong organizational skills and the ability to accomplish assigned work with time constraints.
- Ability to function independently on assigned patient care units and/or outpatient clinics.
- Ability to work as a valued team member, but also autonomously.
Licenses and Certifications:
- CA-RN (Registered Nurse) required
- CDCES - Certified Diabetes Care and Education Specialist required
- Current American Heart Association Certification for Basic Life Support for Healthcare Providers required

Base Pay Scale: Generally starting at $58.72 - $79.33 per hour
