Key Responsibilities\n

• Conduct thorough psychosocial assessments to understand and evaluate the needs of patients, considering their unique circumstances.

• Collaborate with an interdisciplinary team, including educators, medical professionals, and families, to develop a tailored Plan of Care based on each patient’s diagnosis and needs.

• Regularly evaluate and adapt the Plan of Care as needed, responding to changes in patients' conditions and feedback from families and other care providers.

• Participate as an active member of the Two Chairs team, contributing to departmental goals and enhancing organizational efficiency.

• Demonstrate and uphold the highest standards of psychosocial care during field observations by clinical supervisors.

Patient Population / Client Focus\n

In this role, you will engage with a diverse patient population, including individuals of all ages, children, and families from various cultural and socioeconomic backgrounds. You will primarily support those navigating mental health challenges due to chronic health conditions or significant life stressors. Qualifications\n

• A valid and current Licensed Clinical Social Worker (LCSW) license in the state of California is required.

Compensation and Benefits\n

We offer a competitive salary ranging from USD 120,000 to 130,000 annually, commensurate with experience and qualifications.
