Case Manager

Seeking a compassionate Case Manager to support individuals experiencing homelessness in achieving housing stability and self-sufficiency. Ideal candidates are committed to meeting people where they are, utilizing trauma-informed care and harm reduction. Responsibilities Conduct client intakes, assessments, and service planning. Connect clients to housing, healthcare, behavioral health, employment, and community resources. Provide ongoing case management, crisis intervention, advocacy, and housing support. Maintain accurate case notes, client records, and program documentation. Utilize motivational interviewing, trauma-informed care, and harm reduction practices. Collaborate with community partners and multidisciplinary teams. Qualifications A minimum of 1 year of experience in case management, homeless services, healthcare, behavioral health, social services, or related field preferred. Passion for serving vulnerable populations and supporting housing stability. Valid driver's license, reliable transportation, and auto insurance. Ability to pass background check, drug screen, and TB test. CPR/First Aid certification or willingness to obtain.
