Work Environment

This full-time position requires an onsite presence at our hospital facility, where you will work alongside a team of healthcare professionals dedicated to improving patient care standards. With a schedule that runs Monday, Wednesday, and Friday from 8:00 a.m. to 5:00 p.m., you can enjoy a balanced work-life dynamic, as there are no on-call duties or weekend shifts required. Key Responsibilities

• Conduct comprehensive primary care services, performing thorough health assessments and evaluations to ensure a complete understanding of patient needs.
• Perform onsite health risk assessments that identify potential health issues, collaborating with patients to develop effective and personalized intervention plans.
• Provide insightful patient education regarding wellness strategies, preventive measures, and lifestyle choices that enhance overall health.
• Support patients in making healthy lifestyle changes, fostering a proactive approach to personal health management.
• Assist individuals in managing chronic conditions through tailored treatment plans and consistent monitoring to achieve optimal disease management outcomes.
• Coordinate care with medical staff within our facility, ensuring that patient needs are met efficiently and effectively.
• Build collaborative relationships with both clinical and administrative staff to create an open dialogue that enhances overall service delivery.
• Participate actively in health promotion activities, including wellness events and community fairs, to engage the community and foster awareness about health education.

Patient Population / Client Focus

You will serve a diverse patient population comprising individuals and families in the Austin area. Your focus on preventative care and personal relationships with patients will be integral in fostering a culture of health and wellness across various demographics, including children, adults, and seniors. You will have the opportunity to engage patients in meaningful discussions about their health and guide them on their wellness journey.

Qualifications

• A Master’s degree in Nursing with a specialization as a Family Nurse Practitioner is required.
• A minimum of 1 year of experience practicing as a Family Nurse Practitioner in a primary care or family practice setting is essential.
• Current and unrestricted state licensure as a Nurse Practitioner.
• Experience with Electronic Medical Records (EMR) is necessary to streamline workflow and improve patient documentation efficiency.
• Candidates should possess a strong belief in the impact of quality care and effective communication to foster trust and lead wellness initiatives successfully.

Preferred Experience

• Prior experience in a family practice or primary care facility where a patient-centered approach was emphasized.
• A background in conducting health risk assessments and developing community health programs.

Compensation and Benefits
