Position Summary:

Decatur West Personal Care seeking a full-time Qualified Medication Administration Person (QMAP) to join our care team. The ideal candidate is a detail-oriented team player with a solid understanding of Colorado's assisted living regulatory framework — including CDPHE Chapters 2, 7, and 24 under 6 CCR 1011-1 — and a genuine passion for serving older adults. This position is essential to the safe, compliant, and compassionate delivery of care to our residents.

Responsibilities:

• Administer medications accurately and on schedule in accordance with physician orders, resident care plans, and Colorado QMAP standards, with strict adherence to 6 CCR 1011-1, Chapter 7 (Medication Administration) requirements.
• Maintain complete, accurate, and timely Medication Administration Records (MARs); document any refusals, errors, or changes in resident condition per facility protocol and state regulations.
• Monitor residents for medication side effects, adverse reactions, or changes in health status; report findings promptly to the supervising nurse or administrator in compliance with Chapter 24 (Assisted Living Residences) incident and reporting requirements.
• Assist residents with activities of daily living (ADLs) — including bathing, dressing, grooming, toileting, and mobility — in a manner consistent with Chapter 2 (General Licensure Standards) and resident rights provisions.
• Ensure all care activities respect and preserve resident dignity, privacy, and rights as defined under Colorado assisted living regulations.
• Respond to emergencies calmly and effectively; administer first aid and CPR as needed and document incidents per CDPHE reporting requirements.
• Communicate clearly and professionally with residents, families, care team members, and administrators regarding resident health, care, and regulatory compliance matters.
• Maintain a clean, safe, and organized care environment in compliance with CDPHE facility standards and infection control protocols.
• Follow all HIPAA privacy protocols and maintain strict confidentiality of resident records and health information.
• Assist with meal service, hydration monitoring, and nutritional observation as required.
• Support resident engagement through participation in social, recreational, and therapeutic programming.
• Participate in all required staff meetings, mandatory in-service training, and continuing education programs as required by CDPHE and facility policy.
• Stay current with updates to 6 CCR 1011-1, Chapters 2, 7, and 24 and apply regulatory changes to day-to-day care practices.

Qualifications:

• High school diploma or GED equivalent required.
• Current QMAP certification required, or willingness and demonstrated ability to obtain QMAP certification within a defined timeframe following hire.
• Current First Aid and CPR/AED certification required, or willingness and ability to obtain within a defined timeframe following hire.
• Working knowledge of CDPHE 6 CCR 1011-1, Chapters 2, 7, and 24, or willingness to learn and apply these regulations as a condition of employment.
• Strong attention to detail, particularly in medication administration, documentation, and regulatory compliance.
• Proven ability to work cooperatively and effectively as part of an interdisciplinary care team.
• Genuine compassion for seniors and an unwavering commitment to person-centered, dignified care.
• Ability to accurately read, interpret, and follow written care plans, physician orders, work instructions, and regulatory requirements.
• Strong interpersonal and communication skills; consistently professional and respectful in all interactions with residents, families, and colleagues.
• Ability to maintain strict confidentiality and handle sensitive resident information in full compliance with HIPAA.
• Prior experience in an assisted living, memory care, skilled nursing, or home health setting is preferred but not required.
  

Required Certifications and Training (must be current or obtained within a defined timeframe after hire) :

• QMAP — Qualified Medication Administration Person (Colorado-mandated for medication administration in assisted living)
• First Aid Certification
• CPR/AED Certification
• Mandatory Reporter Training — required by Colorado law for all direct care staff working with at-risk adults
• CDPHE Regulatory Training — Chapters 2, 7, and 24 of 6 CCR 1011-1
• Additional training as required by CDPHE, facility policy, or accrediting bodies

Regulatory Knowledge Required:

Candidates must be familiar with — or commit to becoming proficient in — the following Colorado CDPHE regulations governing assisted living residences:

• Chapter 2 — General Licensure Standards: facility operations, staffing requirements, and general compliance expectations.
• Chapter 7 — Medication Administration: QMAP standards, medication storage, administration procedures, error reporting, and documentation requirements.
• Chapter 24 — Assisted Living Residences: resident rights, admission and discharge standards, care planning, incident reporting, and physical environment requirements.

Physical Requirements:

• Ability to lift and assist residents weighing up to 50 lbs., with or without assistive equipment
• Ability to stand, walk, bend, reach, and perform sustained physical tasks throughout an extended shift
• Ability to respond quickly and effectively in emergency or crisis situations
• Ability to work in a residential care environment with potential exposure to various health conditions and infectious materials, with appropriate PPE

Background Check and Compliance:

• All applicants must successfully pass a Colorado Adult Protective Services (APS) background check, including the CDPHE Background Check Program, as mandated for direct care workers in licensed assisted living facilities.
• Checks will include criminal history, Colorado adult abuse/neglect registry, and sex offender registry review.
• Drug screening may be required per facility policy.
• Compliance with all applicable CDPHE regulations, facility policies, and resident rights standards is a condition of continued employment.
