Case Manager, Reentry & Employment Services

The Fortune Society in Bronx, NY is seeking a Case Manager for the CRED program. You will provide community-based case management, assist participants with wrap-around services, and support job readiness and placement activities. You will maintain caseloads, conduct assessments, and perform outreach up to two years post-workshop completion, ensuring accurate documentation and collaboration with internal teams.
