Intake Coordinator
We're hiring an Intake Coordinator to join our Falls Church office. You'll be the first point of contact for new patient referrals — handling intake from physicians and families, verifying insurance, coordinating with our clinical team to schedule starts of care, and making sure every patient gets a smooth transition into home health. This role is a good fit for someone with home health intake experience who wants to be the operational backbone of a busy office.
Handle new patient referrals with urgency and professionalism
Coordinate with physicians, families, and referral sources to gather intake information
Verify insurance and secure necessary authorizations
Collaborate with clinical staff to schedule start-of-care visits
Document referrals and authorizations accurately in WellSky
Serve as the daily point of contact between referral sources and our clinical team
2+ years of home health intake experience preferred
Strong knowledge of insurance verification and Medicare/Medicaid authorizations
WellSky experience is a plus but not required
Proficient with Microsoft Office (Outlook, Word, Excel)
Strong multitasking, communication, and organizational skills
Must be able to work on-site at our Falls Church office
