Key Responsibilities Conduct comprehensive home health risk assessments for Medicare Advantage members in locations such as Palatka, Interlachen, Crescent City, and other areas within Putnam County Perform detailed evaluations of patients’ medical history, current conditions, and medications Educate patients and caregivers about medical concerns, disease management, and prescribed therapies Identify, document, and communicate gaps in healthcare or wellness Offer clinical recommendations to support care coordination—without responsibility for making treatment plans or prescribing medications Qualifications Active Nurse Practitioner license in Florida Board Certification through AANP or ANCC Master of Science in Nursing (MSN) from an accredited institution Current CPR certification (BLS) At least 6 months of recent Nurse Practitioner experience is preferred Strong interpersonal and communication skills; ability to work independently in a home health setting This role provides competitive compensation, with $80–$90 for each assessment completed, a stipend for mileage, and paid administrative time.
