Home » Transition Coordinator The Lakeland Centre for Fetal Alcohol Spectrum Disorder (LCFASD) has been providing Diagnostic, Intervention and Prevention of FASD services since 2000. We are known across the province and nationwide for our novel, non-judgmental, supportive services. Our Mission: To establish and ensure that accurate information about FASD, and effective prevention, diagnosis and support services are available in the region. About The Role The Transition Coordinator is a member of the LCFASD team, with specific direction in transition services for youth diagnosed with an FASD. Under the authority of the Executive Director, the Transition Coordinator assumes responsibility for carrying out assigned duties. The Transition Coordinator will perform all duties while maintaining the integrity of the LCFASD, confidentiality and all other policies outlined in the Policy manual. Key Responsibilities Determine the program opportunities in adult services for individuals with FASD Develop a comprehensive process for youth and families to transition to adult services in the least complicated manner Work with youth diagnosed with FASD to better understand their needs Work to establish relationships with youth and adult services to better meet the needs of young adults with FASD entering Review LCFASD files to determine statistical information on the number of youth needing services and contacting to see if they need assistance with Maintain statistical information on youth Keep community agencies resourced with FASD Support agencies through the transition processAssist LCFASD in determining when youth clients should be transferred to an adult case manager Work with other agencies to learn what services are available in the Develop a good working knowledge of programming, educational opportunities, and services outside the LSA in order to assist families and LCFASD Responsible to build strong working relationships/partnerships Communicate and work in collaboration with key stakeholders Provide education and training Qualifications What you bring to LCFASD: High school diploma. Minimum three years of direct community-based experience working with high-risk populations. Must be culturally competent. Verbal and written communication skills, ranging from individual case management to collaborating with community agencies and speak in front of large groups. Assets Post-secondary education in human services or related field will be considered an asset. Knowledge of applying relational theory to client care considered an asset. Knowledge of FASD will be considered an asset. Skills Non-judgmental, open, honest & willingness to learn about diverse cultures and communities. Creative problem-solving skills. Verbal and written communication. Advocacy. Positive role modelling. Flexibility. Conditions Of Employment Criminal Record Check (with Vulnerable Sector Screening). Child Intervention Record Check. A current Class 5 driver’s license, satisfactory driver’s abstract and good insurance history is necessary to be insured by LCFASD, as transportation of clients is required This position will require driving and use of personal vehicle when the LCFASD vehicle is not available.
