If you're a skilled nurse who thrives on independence and loves connecting with patients in their own environment, this is the opportunity for you!Responsibilities:Conduct comprehensive in-home patient assessments to determine care needs and eligibilityDevelop and document individualized care plans based on assessment findingsEvaluate patient health status, functional abilities, and home environment safetyCollaborate with physicians, care teams, and family members to coordinate appropriate servicesEnsure accurate and timely clinical documentation in compliance with regulatory standardsMonitor and reassess patient conditions, adjusting care plans as neededServe as a patient advocate, connecting clients with community resources and support servicesRequirements:Active and unrestricted Registered Nurse (RN) license in the state of FloridaPrior experience in home health, case management, or community health nursing preferredStrong clinical assessment and critical thinking skillsExcellent communication and interpersonal skills with patients and familiesAbility to work independently and manage a flexible schedule in the fieldReliable transportation and valid driver's license requiredProficiency with electronic health records (EHR) and clinical documentationAbout Us:ComForCare Home Health Care has been a trusted name in compassionate, client-centered home care services, helping individuals live their best lives at home.
