Occupational Therapist, Acute Experience: 2 years Licenses: OT-FL(Copy needed) Certifications: OTR-NBCOT(Copy Needed)

BLS-AHA(Copy Needed) SSN Required DOB Required Work History: Should not be more than 90 days of gap in last 1 year
