Key Responsibilities\n

As a Licensed Professional Counselor at Crestwood Futures Counseling, your primary responsibilities will include:

• Conducting comprehensive assessments that identify and address the unique needs of each patient, allowing for tailored therapeutic interventions.

• Regularly communicating with supervisors, direct care staff, families, and other involved individuals to provide updates and insights on patient progress.

• Submitting structured schedules detailing your appointments, home visits, and monitoring tasks to demonstrate your proactive approach to management.

• Coordinating in-home visits as required, which may include collaborating for chart reviews, conducting staff interviews, and performing data analysis to offer timely solutions.

• Ensuring that all program plans are not only understood but effectively implemented to produce positive patient outcomes.
\n
• Participating in Interdisciplinary Team Meetings and contributing to discussions that inform treatment strategies and improve outcomes.
\n
• Documenting and completing insightful monthly and quarterly progress notes to track and communicate patient developments.
\n
• Organizing and leading training sessions for staff to ensure proper implementation of treatment plans and documentation, bolstering the team’s capability.
\n
• Providing follow-up support through interviews and home visits to continuously address individual patient needs.
\n
• Maintaining comprehensive records to ensure transparent communication and access to information for stakeholders.

• Acting as a liaison to the community and public mental health sector, promoting developmental goals and collaborative practices within mental health resources.

• Contributing to the creation of procedural training materials to enhance program quality and service delivery.

Qualifications\n

Ideal candidates for this position will meet the following qualifications:

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.

• A minimum of two years of experience counseling in a clinical setting is required.

Compensation and Benefits\n
