Key Responsibilities\n

• Conduct thorough assessments and structured interviews to evaluate the needs of clients and develop personalized treatment plans.

• Facilitate effective communication with clients, family members, supervisors, and multidisciplinary team members regarding client progress and treatment goals.

• Schedule and coordinate therapy sessions and home visits, ensuring services are delivered in a timely manner according to client needs.

• Participate actively in Interdisciplinary Team Meetings, contributing insights and recommendations based on your expertise and experiences.

• Organize and conduct training for staff on the effective implementation of treatment plans and documentation standards.

• Maintain detailed, confidential client records to ensure accurate data is available for ongoing assessments and treatment adjustments.

Qualifications\n

Compensation and Benefits\n
