What You’ll Do As a private duty nurse on our home health team, care for medically complex patients, primarily children, where they are most comfortable: at home. Provide personalized nursing care for pediatric and/or adult patients with complex needs. Coordinate care plans with physicians, therapists, and other team members. Monitor patient progress and update care plans as needed. Educate and support families with in-home care techniques. Document visits and care notes in our EMR (electronic charting system). Collaborate with your clinical supervisor and participate in case conferences. Be a trusted part of a long-term care team focused on safety, comfort, and continuity. What You Bring Active RN license. We specialize in long-term adult and pediatric private duty nursing, personal care services, and family caregiving programs, all with a personalized, patient-centered approach. Responsibilities and Duties Registered Nurse (RN) administers skilled nursing care to patients in their place of residence per physician orders and the plan of care. Duties include: Develops, implements, revises, and evaluates individualized patient-centered care plans under physician supervision. Provides case management to an assigned group of patients and ancillary staff. Understands and submits required data collection sets (OASIS, PAR, etc.) and other documentation as requested. Make the initial evaluation visit and reevaluate nursing needs as required by state and payors. Advocate for patient welfare and coordinate care among patients, families, agency staff, and other providers. Provide services requiring specialized nursing skills according to competency level. Demonstrate problem-solving and interpersonal communication skills; identify patient care preferences and ensure interventions meet goals. Initiate preventive and rehabilitative nursing procedures; prepare progress notes and visit summaries. Attend case conferences as assigned; inform physician of changes in condition; counsel patients and families on home health strategies. Educate on home health practices and utilize community resources as applicable. Observe and report symptoms, reactions to treatments, and changes in condition; perform medication reconciliation. Participate in in-service programs; supervise and teach other nursing personnel as assigned. Administer medications per policy and orders; provide written instructions for aides and other staff. Manage documentation, referrals to specialists, and ensure adherence to policies (Infection Control, Compliance, etc.). Participate in QA/QAPI activities; maintain confidentiality and report infections or incidents; on-call duty as assigned. Required Skills/Abilities/Knowledge Average computer skills with competency in media communications. Physical ability to perform duties and supervise ancillary staff as required. Time management, organization, and reliable transportation as needed. Ability to think critically, follow physician orders, and document nursing activity accurately. Alignment with the agency’s philosophy and goals; initiative and independent judgment. Ability to meet health clearance requirements and maintain licensure and continuing education. Excellent verbal and written communication. Education/Experience/Licenses/Certifications Graduate of an approved school of professional nursing and currently licensed in the state(s) of practice.
