PRN Pharmacist

On Call PharmacistLongevity Scripts is seeking to fill an on call pharmacist for our Ft. Myers location. This position will evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability.Essential functions include making recommendations for therapy changes to the physician and facility staff, conducting drug utilization review (DUR) on all prescriptions, reviewing clinical edits to ensure there is no adverse drug interaction, maintaining patient/resident database, reconciling controlled substances log with inventory on hand and prescriptions dispensed to comply with state and federal regulations, providing clinical expertise by responding to drug-related questions and concerns, and documenting all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical record. Schedule will include rotating weekends and/or nights.Minimum qualifications include a Doctorate of Pharmacy, one to three years LTC pharmacy experience, current licensure in the State of Florida, and strong organizational skills including the ability to prioritize and manage multiple tasks in a dynamic environment. On call staff must work at least one shift per month.
