LPN Continuing CareUnder the direction of an RN, mid-level provider, or physician, you will be responsible for assessing, planning, implementing, and evaluating patient care. You will be a vital part of the team, making a real difference in the lives of patients and their families. You will collect and monitor assessment data, reporting the physiological, psychological, sociocultural, and spiritual needs of patients and families. You will provide direct patient care under the direction of an RN, mid-level provider, or physician, consistent with policies and procedures, and unit-based routines. You will inform patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate. You will document care provided in a manner consistent with guidelines. You will greet and communicate with patients and families with care and respect. You will collaborate with other members of the healthcare team to ensure continuity of care and coordination of service. You will maintain a clean patient environment and communicate any equipment malfunctions to appropriate personnel. You will document patient chargeable items according to standards. You will participate in selected quality monitoring activities under the direction of an RN, mid-level provider, or physician. You will demonstrate a high standard of moral and ethical behavior. You will recognize ethical behaviors and identify resources for resolution as needed. You will advocate for the privacy, confidentiality, and security of patient and staff data. You will actively participate in safety initiatives and complete all position-related safety requirements in a timely manner. You may be required to perform other duties as assigned.
