We are currently seeking an experienced Home Health Registered Nurse (RN) for a travel assignment in California. This position is ideal for clinicians who enjoy independent field-based work while providing compassionate care to patients transitioning from hospital to home.

Job Responsibilities:

• Provide skilled nursing care to Home Health patients in residential settings
• Perform Start of Care, Recertification, Resumption of Care, and routine nursing visits
• Educate patients and families on disease management, medications, wound care, and medical equipment
• Care for patients with diagnoses including CHF, COPD, wounds, wound vacs, and IV/PICC lines
• Complete accurate documentation and care planning
• Coordinate with interdisciplinary healthcare teams to ensure continuity of care
• Maintain productivity expectations and daily visit schedules

Requirements

• Active RN License
• Minimum 2 years of Home Health or relevant nursing experience required
• Current BLS Certification required
• Valid driver’s license and active auto insurance required
• HomeCare HomeBase (HCHB) experience preferred
• Strong assessment, communication, and time-management skills
• Ability to work independently in the field
