THP Plus Social Worker: Youth Housing & Life Skills Coach
Walden Environment
Walden Family Services is seeking a THP PLUS Social Worker to partner with youth, families, and placing agencies to help youths achieve treatment goals. The role emphasizes thorough intake, needs and services planning, and ongoing support through weekly consultations and coordinated referrals.
Responsibilities include maintaining accurate records, providing crisis intervention as needed, and facilitating trainings.
