@Home Therapy is looking for a Speech-Language Pathologist to provide in-home services to adult patients in the Hempstead area. This role focuses on helping individuals improve communication, cognitive function, and swallowing abilities-right where they feel most comfortable.

Your Impact in This Role:

Communication & Cognitive Therapy:

• Evaluate and treat speech, language, and cognitive-communication disorders
• Support patients with memory, attention, and problem-solving challenges

Swallowing & Safety Management:

• Assess and treat swallowing disorders (dysphagia)
• Provide recommendations to improve safe eating and reduce aspiration risk

Personalized Care Delivery:

• Develop therapy plans tailored to each patient's home environment and goals
• Modify interventions based on progress and clinical findings

Patient & Caregiver Education:

• Teach strategies that patients and caregivers can apply between visits
• Promote long-term carryover of communication and swallowing techniques

Documentation & Coordination:

• Complete visit notes using a digital EMR system
• Communicate with care team members as needed to support patient outcomes

Qualifications:

• Active New York State Speech-Language Pathologist license
• Experience with adult populations preferred
• Knowledge of dysphagia and cognitive-communication treatment

• Comfort working in a mobile, home-based setting
