Job Summary Ambulatory Oncology Case Manager / RN Care Coordinator – Full Time, 40 Hours, Variable, Day Shift at MGB Cancer Institute (Main Campus), Brigham & Women's Hospital, Inc. The ambulatory oncology case manager is a vital link in providing patients with access to services and treatments needed along the cancer care journey, including coordination of home services, prior authorizations, and documentation in the electronic health record. The role collaborates with physicians, APPs, ambulatory practice RNs, pharmacies, outside vendors, patients, and families. The candidate will have foundational oncology care knowledge, strong patient advocacy, and exemplary professional communication skills. Essential Functions Perform program‑specific patient outreach, education, and recruitment. Consult with patients’ physicians and/or Medical Directors about high‑priority patients and potential care management plans. Initiate telephone or in‑person contact with eligible patients to perform initial assessment, healthcare education, and develop a realistic care management plan. Incorporate knowledge of case management, levels of care, and utilization management principles to implement high‑quality, cost‑effective care plans. Communicate with patients, families, and physicians to build rapport, trust, and engage patients in health promotion activities. Influence utilization of health‑care resources by providing direct care coordination to patients, encouraging enrollment in disease and case management programs, and providing follow‑up prior to and after interaction with the health‑care system. Determine appropriateness of hospital admissions and disease management programs using medical management or diagnostic screening criteria. Communicate with other health‑care clinicians throughout the continuum about patient care, utilization, and follow‑up plans. Establish a consistent communication and reporting schedule for periodic contact with providers and patients to review progress toward goals. Notify Nursing Director, PCP, MD Advocate, and/or Medical Director about utilization and patient compliance. Collaborate with PCP, MD Advocate, and/or Medical Director on challenging patient situations. Act as clinical resource person for the program’s quality efforts. Attend and present case reviews at practice meetings, program meetings, and care coordination meetings. Utilize care management systems to document, monitor, and evaluate patient interventions and care plans. Keep current with related trends in care management, including health education and coaching. Participate in regular meetings with the Nursing Director and Medical Director to review performance, patient volume, projects, outside professional activities, and upcoming goals. Demonstrate a positive attitude in dealing with problems or crisis situations. Follow BWH policies and procedures for safety, fire safety, parking, body mechanics, infection control, attendance, punctuality, and appearance. Work effectively with team members and demonstrate initiative and creativity to continuously improve services, processes, and quality. Perform all duties in an independent, professional manner and request assistance when necessary. Show excellent organizational skills and flexibility to cover duties of lesser, equal, or greater responsibility as requested. Assume accountability for professional growth and development. Exemplify program teachings and act as a role model for patients. Assist in preparation for Joint Commission, CMS, and other surveys as applicable. Work within legal, regulatory, accreditation, and ethical practice standards relevant to the position. Follow safe practices required for the position and comply with BWH and Partners policies and procedures. Fulfill any training required by BWH and/or Partners, as appropriate. Bring potential non‑compliance issues to the attention of the supervisor or other appropriate hospital staff. Qualifications Education: Bachelor’s Degree in Nursing required; Master’s Degree in Nursing preferred. BSN required for newly licensed nurses. Experience in lieu of a degree is accepted. Licenses and Credentials: Current licensure as Registered Nurse in the Commonwealth of Massachusetts required. Experience: Minimum 1–3 years of oncology care experience within the past five (5) years required. Minimum 1 year of case management experience in an outpatient setting within the past five (5) years required. Experience with complex patient populations (e.g., solid tumors, hematologic malignancies, bone marrow transplants) strongly preferred. Health‑care reimbursement experience strongly preferred. Knowledge of insurance authorization processes and healthcare systems strongly preferred. Knowledge, Skills and Abilities: Excellent organizational and communication skills. Excellent oral, written, and telephonic skills and abilities. Superior inter‑personal skills. Strong competency working with hospital computer systems and case management systems. Competent using Microsoft Word, PowerPoint, and Excel. Ability to work well with physicians and ambulatory staff in a practice or health center setting. Demonstrated ability to present and speak in front of groups. Knowledge and skills to differentiate levels of care. Ability to handle routine work, unexpected priorities, and multitask. Flexible about changing practice sites as patient needs indicate, helping colleagues at other practices and covering during vacations/unexpected illness/holiday time. Familiarity with various forms of health‑care reimbursement.
