Details Field-based Home Health RN role providing in-home patient care within a designated service territory Day shift schedule, Monday through Friday, 8:00 AM to 5:00 PM Average of 4-6 patients per day, with a minimum requirement of 30 points per week Patient population includes adult and geriatric home care cases Responsible for interdisciplinary care coordination and plan of care development and oversight Charting must be completed using HCHB and OASIS while at the patient's home Weekend coverage required as needed Participation in on-call rotation required, including at least one weeknight per week Must be willing to travel up to 50 miles outside the home base office Requirements Active TX or compact RN license required, must be in hand at time of consideration Minimum 1 year of Home Health nursing experience required Minimum 2 years of overall clinical nursing experience required BLS certification required HCHB and OASIS charting experience required Valid driver's license required Must have reliable transportation and insurance Non-smokers preferred due to patient sensitivity to smoke odor Additional Information Responsible for administering skilled nursing care to patients from diverse populations across a variety of home environments Coordinates care with patients, families, certifying providers, and interdisciplinary team members Performs comprehensive patient assessments and collaborates on plan of care development On-call responsibilities include taking after-hours calls and completing related charting Dress code consists of scrubs in any color First-time travelers are welcome to apply Candidates must reside within a 50-mile radius of the home base office to be considered local Mileage reimbursement provided at the IRS standard rate, calculated from the first to the last patient visit American Traveler Job ID P-746372.
