LPN - LTC

Tryfacta is seeking an LPN - LTC for our client in Burlington, VT, 05401. This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please apply now!

Location: Burlington, VT, 05401

Duration: 13 weeks

Work Schedule: ASAP (Tuesday Start Only)

Responsibilities for this position include, but are not limited to:

• Provide direct nursing care to residents in a Long-Term Care/Rehabilitation setting.
• Administer medications and treatments as prescribed.
• Monitor, assess, and document resident conditions using PointClickCare (PCC).
• Collaborate with the interdisciplinary team to ensure quality resident care.
• Assist with admissions, discharges, and care planning.
• Maintain compliance with facility policies and nursing standards.

To be considered for this position, you should have:

• Current and valid Vermont LPN License (or eligibility to practice in Vermont).
• BLS Certification required.
• Previous LTC/SNF/Rehabilitation experience preferred.
• Experience with PointClickCare (PCC) is a plus.
• Strong communication, teamwork, and patient care skills.
