Responsibilities
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team.
- Contribute to a developmentally appropriate, culturally relevant care plan reflecting nursing diagnoses and interventions; involve resident, family, and healthcare team.
- Recommend revisions to the plan of care after evaluating resident outcomes and coordinate referrals as needed.
- Provide direct and indirect skilled nursing care under RN supervision; maintain collaboration with nursing staff, physicians, and ancillary staff.
- Maintain current nursing practice knowledge; participate in continuing education and inservice activities; contribute to mentoring or orientation of staff.
- Demonstrate leadership in the professional practice setting and apply safety, cost, and quality considerations in care delivery.

Requirements
- Active NY LPN license
- BLS certification (AHA or RQI required)
- Minimum 1 year of experience as an LPN in a long-term care setting
- Ability to provide care to adult and geriatric residents; understanding of growth and development across the lifespan
- Ability to work in a skilled nursing facility environment under supervision of an RN
- Evidence of ongoing competency and a collaborative approach to care
