Key Responsibilities\n

• Provide individual and group psychotherapy sessions to patients dealing with mental health conditions, such as depression, anxiety, and PTSD.

• Conduct comprehensive assessments of patients’ emotional and social functioning, utilizing standardized tools to gauge mental health status and identify treatment needs.

• Develop, implement, and modify evidence-based treatment plans that are culturally sensitive and reflect the diverse backgrounds of our patient population.

• Collaborate closely with interdisciplinary team members, including psychiatrists and primary care providers, to facilitate holistic and coordinated patient care.

• Monitor patient progress through regular follow-ups, adjusting treatment plans as necessary based on feedback and assessments of outcomes.

• Provide education and guidance to patients and their families about mental health conditions, treatment options, and available community resources.

In this role, you will work primarily with a diverse range of patients in a hospice setting, many of whom face significant mental health challenges due to chronic illness, grief, and end-of-life transitions. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

This position offers a competitive salary range of $90,000 to $105,000 per year, commensurate with experience.
