Responsibilities
- Perform initial evaluations and develop individualized treatment plans to restore physical function and enhance quality of life
- Deliver PT interventions using established treatment protocols and modify plans as patient progress dictates
- Establish and communicate care plans, discharge goals, and progress with patients, families, and the interdisciplinary team
- Complete comprehensive clinical documentation within established deadlines
- Supervise assigned assistants and ensure appropriate supervision and delegation
- Collaborate with the care team to optimize patient outcomes and coordinate transitions of care

Requirements
- Doctor of Physical Therapy (DPT) degree; required as specified (only if license obtained on or after January 2016; or foreign equivalent)
- Physical Therapy licensure in state of practice: Required
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from an approved vendor per policy: Required
- Valid Driver’s License and must be at least 21 years of age: Required
- At least two years of recent physical therapy experience
- Knowledge of HCHB and OASIS
- Current home health experience (at least 1 year) per job notes
- Current BLS/CPR certification
- Ability to work in a hospital/home health setting and manage patient care under a travel assignment
