Physical Therapist (PT) – Home Health

For 50 years, VNA of Florida has provided trusted home health care throughout Florida. We are seeking compassionate LPNs who enjoy working independently while making a meaningful difference in patients' lives.

What You'll Do

Complete in-home physical therapy evaluations
Develop individualized treatment plans
Improve mobility, strength, balance, and function
Educate patients and caregivers
Coordinate care with physicians and clinicians
Maintain clinical documentation

What You'll Need

Active Florida PT License
Strong assessment and treatment skills
Home health experience preferred
Ability to work independently

Ideal For: Physical therapists, orthopedic therapists, rehabilitation clinicians, home health PTs
