Actual compensation may differ depending on assigned hours and position status (e.g., part-time).Pay Range:$67,225.60 - $87,838.40Scheduled Weekly Hours:40Position OverviewESSENTIAL FUNCTIONSConduct comprehensive psychosocial assessments and mental status examinations to formulate DSM-5-TR diagnoses and treatment plansProvide evidence-based counseling and psychotherapy to individuals, groups and families, using advanced expertise in medical/mental health diagnoses, chronic illnesses and medications to meet the needs of a diverse patient populationIdentify patient strengths, coping skills and needs in collaboration with families and interdisciplinary teamsDeliver immediate crisis intervention services and represent patient rights and needs to providers and the communityMaintain accurate medical records, progress notes and treatment plans in accordance with clinic protocols and regulatory agency requirementsMonitor patient outcomes and registries to develop workflows and performance initiatives based on quality metrics and best practicesContinually review and implement regulatory requirements to ensure all social work services and documentation meet mandatory standardsLead planning meetings, case conferences and care team huddles and provide expert consultation on how psychosocial factors impact patient health and treatment complianceServe as a primary liaison between outside agencies, hospital departments and the medical teamMaximize staff scheduling and assist with the recruitment, hiring and onboarding of new clinical social work personnelMeet regularly with LSW and CSW staff to identify and resolve operational issues, providing recommendations to the leadership teamCoordinate staff training and performance improvement initiatives to increase clinical awareness of biological, psychological and social issuesPerform other duties as assignedMINIMUM REQUIREMENTSMaster of Social Work (MSW) required.Five years of independent clinical social work experience in a mental health or hospital-based setting performing medical crisis counseling and discharge planning.
