Social Worker

A rehabilitation and skilled nursing facility in Brooklyn is seeking a Social Worker (SW) to join its care team.

Duties:

• Maintain and/or improve residents’ ability to manage physical, mental, and psychosocial needs

• Identify individual social and emotional needs

• Develop and maintain individualized care plans

• Participate in admission planning, community re-entry, and transfers

• Attend interdisciplinary care plan meetings to support residents’ emotional and social well-being

Requirements:

• Bachelor’s Degree in Human Services, Psychology, Social Work, or related field

• MSW required

• Long-term care or subacute experience required

• Bilingual Chinese-speaking candidates preferred
