Care Coordinator

Case Manager Location: Mineola, NY Schedule: Monday–Friday, 8:00 AM – 4:00 PM ET Compensation: $26.00/hour We are seeking a compassionate and dedicated Case Manager to support elderly and vulnerable populations within the community. This role is ideal for someone passionate about advocacy, care coordination, and helping individuals maintain safety, independence, and quality of life through comprehensive case management services. As a Case Manager, you will work directly with clients and their families to assess needs, coordinate services, provide crisis intervention, and connect individuals with essential community resources and support programs. Key Responsibilities Assessments & Client Support Conduct comprehensive in-home assessments and periodic reassessments of clients and their support systems Evaluate client needs, living conditions, safety concerns, and service gaps Care Planning & Coordination Develop, implement, and monitor individualized care plans in collaboration with clients and family members Coordinate community-based services and referrals to address unmet needs and improve client well-being Benefits & Resource Navigation Screen clients for eligible benefits, entitlements, and assistance programs Facilitate referrals to community resources, healthcare services, and supportive programs Crisis Intervention Respond to urgent client situations including medical crises, housing or environmental safety concerns, utility shut-offs, and other emergencies Provide immediate intervention and coordination of appropriate support services Monitoring & Compliance Maintain regular contact with clients, including those receiving home-delivered meals, in compliance with state regulations and program requirements Monitor ongoing service delivery and client status at minimum every two months Documentation & Administrative Support Maintain accurate, timely case documentation and regulatory records within the electronic case management database Assist with office support duties including answering phones and general administrative tasks as needed Qualifications Required Qualifications Bachelor’s degree in Social Work, Gerontology, Human Services, Sociology, Psychology, Nursing, or a related human services field Valid New York State Driver’s License and reliable transportation for community visits and outreach Ability to lift 30–50 lbs. when necessary during field visits or community activities Ability to complete an online Case Management Certification within five months of hire Commitment to maintaining 16 hours of continuing education annually Strong communication, organizational, and interpersonal skills Comfortable working independently in both office and field settings Preferred Qualifications Previous experience working with elderly, vulnerable, or homebound populations Familiarity with Nassau County senior services, community resources, and geriatric care needs Prior case management or human services field experience preferred Why Join Us?
