Position Overview

Provide diagnostic, therapeutic, and preventive care including initial assessment and ongoing management

Order and interpret diagnostic tests and prescribe medications within scope as outlined in the order requirements

Educate patients and coordinate care with an interdisciplinary team

Required Qualifications

Graduate of an accredited nurse practitioner program

Active Connecticut advanced practice license required prior to start

Active DEA required

Connecticut controlled substance registration required prior to start

Required certifications as outlined in the order requirements

Candidates must have at least two years of clinical experience within the past five years to qualify
