Lao Family Community Development, Inc. (LFCD) is a community-based non-profit organization established in Richmond, Contra Costa County, California, in 1980. Since then, LFCD has expanded its operations to Alameda and Sacramento counties with its headquarters located in Oakland, California.
LFCD delivers programs and services from seven locations in more than 35 languages. The organization provides community development, affordable real estate, workforce development, education, and human services that primarily support low-income individuals and families, refugees, immigrants, transitional-age youth, seniors, individuals with disabilities, and other underserved populations.
Job Summary: LFCD is looking for a candidate to provide housing support services to the re-entry population, including community outreach efforts to form community collaborations, promote the understanding of the program, and build relationships with funders and partners to increase the number of referrals to the program. The candidate will work closely with onsite operational staff to provide ancillary services as needed. The position also manages participants under the California Advancing and Innovating Medi-Cal (Cal-AIM) Initiative, which provides comprehensive care management to address both clinical and non-clinical needs. The position duties include: outreach recruitment for potential landlords and preparing tenants to place in subsidized housing, case management, providing ongoing support to these individuals, and follow-up and retention services. This position will directly report to the Program Coordinator.
Roles and Responsibilities:
- Conduct all program's intake eligibility, verification, obtain all required documents, orientation, assessments, workshops, and peer support coaching
- Build confidence, readiness, and emotional health of participants to address barriers towards stabilizing life and work situations
- Create individual personal goal plans, housing plans, and savings plans; conduct one-on-one or group peer support coaching and financial coaching; assist participants in reaching their personal goals, saving goals, and moving from transitional housing to permanent housing
- Assist clients with providing resources and referral assistance, including mental health counseling, substance abuse counseling, legal assistance, and social adjustment assistance
- Establish and maintain relationships with other service providers, including banks, credit unions, OHA, landlords, and other housing program providers
- Create and manage a landlord's directory for affordable permanent housing placement and support CARE Campus residents with their needs
- Recruit, interview, and negotiate with landlords to assist clients in finding permanent, affordable housing
- Provide landlords with orientations on the project value, goals with target population profile, written agreements, and other technical supports to cultivate, build trust, and maintain business relationships with funders, subcontractor partners, landlords, and other community service providers
- Manage participants enrolled in the Cal-AIM Initiative, providing comprehensive care management that addresses both clinical and non-clinical needs, including housing instability, food insecurity, and other social determinants of health
- Develop and monitor Cal-AIM participant care plans, coordinate with managed care plans and health providers, and ensure participants receive appropriate Enhanced Care Management (ECM) and Community Supports services
- Maintain all program-related documents for monitoring and compliance with federal, state, and local regulations under the project's requirements
- Represent LFCD at local collaborative community meetings, attend all the required training, housing-related meetings, or conferences as assigned
- Attend monthly meetings as needed, participate in "Client Resource Forums", Housing fair, and a visit to the County jail for recruitment
- Maintain case records, statistics, and service records for program reports
- Perform tracking of participants' attendance, input data, daily log, standard case management, and job activities into the database system for record-keeping, and provide follow-up service till they successfully exit the program
- Maintain a thorough understanding of the Cal-AIM program and leverage cross-sector partnerships to support positive housing and health outcomes for participants
- Other duties as assigned
Requirements and Qualifications:
- A Bachelor's degree or equivalent with a concentration in social work, psychology, sociology, or other related fields preferred
- Bilingual in English and Spanish preferred
- Knowledge of the Cal-AIM Initiative, including Enhanced Care Management (ECM) and Community Supports, and experience coordinating services across health and social service systems preferred
- Self-starter, ability to work with minimal supervision, and experience working with multiple barriers and the reentry population
- Good written and oral communication skills. Good organizational skills, attention to detail, and ability to multitask. Good analytic skills, critical thinking skills, ability to create accurate and insightful data outcomes and evaluations, and strong Excel, Word, and PowerPoint presentations
- Proven track record in managing and working independently
- Ability to work late or on weekends as needed for activities related to the program
- In compliance with federal law, all persons hired will be required to verify identity and eligibility to work in the United States and to complete the required employment eligibility verification form upon hire; must pass a background check

While performing the duties of this job, the employee is regularly required to sit and work on the computer; use hands to handle or feel and talk or hear; move objects up to 25 lbs.
Compensation: The hourly rate is based on experience and education, along with a comprehensive benefits package including health plan/vision, dental, paid vacations, holidays, sick leave, and employer-contributed pension/group life insurance.
