Your Valued Contributions: This position combines traditional staff pharmacist responsibilities with ambulatory care clinical pharmacy services to support optimal patient outcomes across the organization. Dispenses pharmaceuticals, monitors inventory, and provides collaborative consultation services to medical staff. This role promotes safe, effective, and cost-efficient medication use by providing medication therapy management, monitoring drug therapies, educating patients and staff, participating in interdisciplinary care teams, and evidence-based decision making. Through this integrated approach, the Clinical Staff Pharmacist helps improve medication safety, enhance clinical outcomes, optimize value-based care performance, and advance Alcona Health Center's mission of providing high-quality, accessible healthcare to the communities we serve. Professional Pharmacy Standards Advocate Assures that the clinic maintains standards of pharmacy practice equal to or above those accepted by professional ethics, and federal/state regulatory agencies. Provides in-service training for pharmacy personnel or other clinic personnel as related to pharmaceutical issues Responsible for All Facets of Prescription Dispensing Performs all functions required in the accurate dispensing of pharmaceuticals, such as reviewing prescriptions written by providers for accuracy and legality; detecting therapeutic incompatibilities. Ensures medication reaches the patient in the correct form and dose - this may include tablets, capsules, ointments, injections, inhalers and creams. Utilizes pharmacy technicians/assistants as appropriate, with the pharmacist assuming final responsibility for work performed by the staff. Reviews medical charts per AHC requirements. Maintains all necessary documentation, including pharmacy files, and updates patient records as appropriate. Provides appropriate instructions/education for pharmacy staff. Complies with state and federal drug laws as regulated by the state board of pharmacy, the drug enforcement administration, and the food and drug administration by maintaining records for controlled substances; removing outdated and damaged drugs from the pharmacy inventory; maintaining current registration. Ensures pharmaceuticals are ordered, stored and dispensed. Provides Pharmacy Expertise to Medical and other Staff Provides appropriate and timely pharmaceutical consultation services to providers, nurses, patients and customers at the pharmacy. Participates in quality assurance programs and projects supporting the vision and mission of the pharmacy and clinic. Assists with training and orientation of new pharmacy employees or students. Provides Clinical Pharmacist Services and Medication Management Services Conducts comprehensive medication reviews and medication reconciliation to identify, resolve, and prevent medication-related problems, including therapeutic duplications, drug interactions, adverse drug events, contraindications, and adherence barriers. Assesses each patient's medication regimen for appropriateness, effectiveness, safety, and ability to be taken as intended, and develops patient-centered medication care plans in collaboration with providers and the care team. Monitors patient response to medication therapy, including clinical outcomes, laboratory values, side effects, adherence, and medication access concerns; recommends therapy adjustments as appropriate. Provides patient education and counseling regarding medication purpose, proper use, dosing, administration, expected benefits, potential side effects, adherence strategies, and when to seek medical attention. Collaborates with medical providers, nurses, care managers, pharmacy staff, and other members of the health care team to support safe, effective, evidence-based, and cost-conscious medication therapy. Documents medication management services, interventions, recommendations, patient education, and follow-up plans in the appropriate pharmacy and/or electronic health record systems. Supports chronic disease management and population health initiatives through medication optimization, adherence support, preventive care services, and follow-up with patients as assigned. Participates in medication safety, quality improvement, formulary management, and regulatory compliance activities related to medication use and medication management services. Strategic and operational collaboration Collaborate with clinic operations, medical leadership, and interdisciplinary care teams to develop, implement, and optimize clinical pharmacy services that support organizational priorities and strategic objectives. Partner with operational leaders to identify opportunities that improve performance under value-based care models, payer contracts, and quality incentive programs. Support the achievement of value-based agreements by implementing evidence-based medication management strategies that improve clinical outcomes, enhance quality metrics, reduce total cost of care, and improve patient experience. Participate in the development and execution of initiatives that align pharmacy services with organizational strategic goals, population health management, and care transformation efforts. Collaborate with care management, quality improvement, and operations teams to standardize workflows, optimize medication-related processes, and improve care coordination across the continuum of care. Contribute to the development clinical protocols, quality improvement initiatives, and performance metrics that support regulatory compliance, accreditation standards, and organizational strategic planning. Ensures quality and safety for related programs, such as Medication Therapy Management, remote dispensing, site medication closets, and sample medications. Maintains pharmacological knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies. Required Training & Experience: Possesses a current Basic Cardiac Life Support (BCLS) CPR. Possesses a current State of Michigan licensure as a Pharmacist. Possesses a current State of Michigan driver's license and valid automobile insurance. Proficient with Microsoft Office suite of products. Effectively use the pharmacy software system.
