Primary Care Physician

Role Overview: The Primary Care Physician (PCP) provides comprehensive care for older adult patients with acute and chronic conditions, ensuring quality outcomes and cost-effective treatment. Responsibilities: Independently assess, diagnose, treat, and manage patient health, collaborating with specialists and care teams for in-hospital, SNF, LTC, or homebound patients. Lead and coordinate care teams, promote health goals, and engage in care transitions. Maintain compliance with clinical guidelines, quality standards, and organizational policies. Support center operations, participate in clinical meetings, and contribute to a positive, patient-centered culture. Leadership & Collaboration: Demonstrate physician leadership, influence positive behaviors, and foster teamwork. Supervise APPs, participate in recruitment, and contribute to center management. Skills & Qualities: Show accessibility, service orientation, evidence-based practice, leadership development, quality focus, influence, and self-care. Proficient in Microsoft Office, adaptable to travel, and fluent in English. Exercise independent judgment and work flexibly, including evenings, weekends, or holidays as needed. Qualifications: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or related, with an active medical license and board eligibility or certification.
