Registered Nurse - Case Management - Hospital

$1591.20/week -

Job Summary:

Seeking an experienced RN Case Manager for a 13-week contract providing inpatient and observation patient care. Responsible for comprehensive psychosocial, medical, and discharge planning to ensure optimal patient outcomes.

Responsibilities:

• 
  Perform thorough assessments of patients' psychosocial, medical, and discharge needs.
  
  
• 
  Develop and coordinate individualized care plans in collaboration with the multidisciplinary team.
  
  
• 
  Utilize InterQual criteria to evaluate care progression and drive discharge decisions.
  
  
• 
  Conduct discharge planning, patient education, counseling, and advocacy services.
  
  
• 
  Monitor patient progress and adapt care plans as necessary, ensuring efficient patient flow.
  
  
• 
  Document all assessments, care plans, and communications accurately within the EMR system.
  
  

Qualifications:

• 
  Hold an active RN license with at least 2 years of acute care case management experience.
  
  
• 
  Demonstrate strong assessment, communication, and critical-thinking skills.
  
  
• 
  Ability to effectively collaborate with team members and build trust with patients.
  
  
• 
  Proven ability to remain calm under pressure and handle high-stress situations.
  
  
• 
  Experience with Meditech EMR and Midas/CAREPORT/InterQual tools preferred.
  
  
• 
  Must be able to work 8:00 AM 4:30 PM, rotating weekends, no call required.
