Key Responsibilities:

• Complete comprehensive home health risk assessments for Medicare Advantage members
• Review and evaluate prior and current medical history, medical conditions, and all medication usage
• Educate members on their conditions, medications, and promote health literacy
• Identify potential gaps in care and make recommendations to address them
• No duties involving diagnosis, treatment plan creation, or prescribing medications

Qualifications:

• Unencumbered Nurse Practitioner license in Missouri (MO)
• Current CPR (BLS) certification required
• Board certification by AANP or ANCC
• Graduate of an accredited MSN program
• At least six months of recent nurse practitioner experience preferred

This position offers an evaluation rate of $80-$90 per visit, with an additional mileage stipend and compensation for administrative time.
