Responsibilities
- Plan and conduct segments of physical therapy treatment programs to restore and improve physical function, mobility, and daily living activities.
- Perform comprehensive evaluations and establish individualized treatment plans with clear goals and timelines.
- Deliver therapy interventions using appropriate techniques and protocols; adjust plans based on patient progress.
- Collaborate as a co-leader within an interdisciplinary care team; communicate progress and discharge plans to patients, families, and care colleagues.
- Develop and communicate individualized discharge plans; ensure documentation is accurate and timely.
- Supervise assigned therapy assistants and ensure adherence to safety, regulatory, and documentation standards.

Requirements
- Education: Doctor of Physical Therapy (DPT) degree; required, with note: only if license was issued on or after January 2016; or foreign equivalent.
- Licensure: Active Physical Therapy license in the state of practice (California) or eligibility.
- Experience: Two years of Physical Therapy experience preferred.
- Certifications: CPR or Basic Life Support (BLS) certification from an approved vendor; required.
- Professional systems: EPIC electronic medical record (EMR) experience required.
- Additional: Ability to work in a hospital setting and meet essential functions of the role.
