Director Pharmacy, Transition of Care; Mount Sinai Health System; Full Time; Days

Job Description Director of Pharmacy - Transitions of Care (TOC) Provides strategic and operational leadership for enterprise-wide pharmacy services that ensure safe, effective, and coordinated medication management as patients transition across the continuum of care, including admission, inpatient care, discharge, ambulatory follow-up, and community pharmacy integration. This role is responsible for developing, implementing, and optimizing pharmacy‑led TOC programs that improve medication safety, patient outcomes, readmission performance, patient experience, and medication access while aligning with health system priorities for value‑based care, population health, and quality improvement. The Director collaborates with clinical, operational, informatics, ambulatory, and community partners to ensure medication continuity and coordinated care across the health system. Responsibilities Strategic Leadership Develop and execute a patient centered enterprise Transitions of Care pharmacy strategy aligned with health system clinical quality and population health priorities. Lead system‑wide initiatives to reduce 30‑day hospital readmissions, medication‑related adverse events, and post‑discharge medication errors. Establish pharmacy as a core clinical leader in care coordination and discharge planning workflows. Align TOC services with value‑based care programs, CMS quality metrics, and Joint Commission standards and Leapfrog. Program Development & Operations Design and implement pharmacy‑led TOC programs across inpatient, ambulatory, specialty, and retail pharmacy settings. Develop standardized workflows for: Admission medication reconciliation Discharge medication reconciliation Medication counseling Meds‑to‑Beds programs Medication access support Post‑discharge follow‑up calls Referral to Ambulatory Pharmacy Services Implement disease‑specific TOC bundles for high‑risk populations such as: Heart Failure COPD Diabetes Anticoagulation Sepsis and complex polypharmacy patients. Integrate pharmacy services into interdisciplinary discharge planning and care coordination teams. Establish and monitor Key Performance Indicators such as: Medication reconciliation completion rates Medication discrepancy reduction Readmission rates Medication adherence and persistence Clinical Program Oversight Establish clinical standards for: Medication reconciliation Medication therapy optimization High‑risk medication management Medication access and affordability programs Oversee patient counseling and medication education initiatives to improve adherence and health literacy. Ensure clinical pharmacists are integrated into interdisciplinary rounds and discharge planning processes. Education & Professional Development Develop training programs for pharmacists, technicians, residents, and students in TOC best practices. Serve as a preceptor for pharmacy residents and students. Promote scholarship, research, and publication related to transitions of care initiatives. Regulatory & Compliance Ensure TOC programs meet regulatory standards including: Joint Commission medication management standards CMS readmission reduction programs Medication safety requirements Develop policies and procedures for medication continuity across care settings. Qualifications Education Doctor of Pharmacy (PharmD) or Advanced Pharmacy degree required Training PGY1 Pharmacy Residency required PGY2 Residency in Ambulatory Care, Health‑System Pharmacy Administration, or related field preferred Licensure and Certifications Active NYS Pharmacist License Board Certification (BCPS, BCACP, or equivalent) preferred Experience Minimum 7?10 years of progressive pharmacy leadership experience Demonstrated experience developing or managing Transitions of Care or population health programs Experience in health system pharmacy operations and interdisciplinary care coordination Core Competencies Strategic healthcare leadership Population health management Care coordination and discharge planning Clinical program development Data‑driven quality improvement Interdisciplinary collaboration Health system operations Change management Non‑Bargaining Unit, 600 - HSO Pharmacy - MSH, Mount Sinai Hospital Equal Opportunity Employer The Mount Sinai Health System is an equal opportunity employer, complying with all applicable federal civil rights laws.
