Key Responsibilities\n

• Conduct thorough psychosocial assessments that identify patients' emotional, social, financial, and support needs upon admission and throughout their treatment journey.

• Participate in multidisciplinary team meetings, contributing valuable insights from a social work perspective to optimize holistic care plans and ensure a comprehensive understanding of patients’ needs.

• Provide individual and group counseling focused on addressing adjustment disorders, grief, trauma, and coping strategies for substance use challenges, fostering a safe environment for emotional expression.

• Coordinate with community resources and organizations to facilitate seamless post-discharge planning, ensuring that patients have access to the necessary support systems for ongoing recovery.

• Offer educational resources and emotional support to families, empowering them with knowledge on treatment options and post-treatment recovery strategies.

• Engage in crisis intervention as needed, providing immediate support and options to patients experiencing acute psychological distress.

• Maintain precise and up-to-date documentation, adhering to hospital policies and regulatory standards.

Patient Population / Client Focus\n

You will primarily work with individuals experiencing substance use disorders, alongside co-occurring mental health conditions. Our patient demographic is diverse, including varying age groups, cultural backgrounds, and socioeconomic statuses. By understanding and addressing the unique contexts of each patient’s life, you will be instrumental in tailoring interventions that resonate and foster recovery.

Qualifications\n

• A Master's degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) in California.
