Work Environment

This role will be performed onsite within patient homes, allowing you to deliver personalized care in a comfortable setting for your clients. Key Responsibilities

• Conduct comprehensive examinations and evaluations of patients with functional limitations and disabilities, formulating individualized treatment plans that reflect evidence-based practice.
• Provide consultation regarding the continuum of care for each patient in collaboration with healthcare providers, ensuring a holistic approach to recovery.
• Implement treatment protocols, continuously re-evaluating patients' progress and modifying plans based on their responses to treatment.
• Educate patients and their families about the treatment process,-home exercises, and lifestyle modifications that support recovery.
• Maintain precise documentation of evaluations, treatment notes, progress reports, and billable services in compliance with all applicable regulations and organizational standards.
• Engage in community outreach activities, participate in quality improvement initiatives, and contribute to program development to enhance service delivery.
• Attend regular staff meetings and commit to continual professional development through in-services and educational opportunities.

Patient Population / Client Focus

You will be serving a diverse patient population in the Charlottesville area, focusing on individuals recovering from orthopedic surgeries, accidents, chronic pain, and other conditions impacting mobility and functionality. Your compassionate approach will be essential in understanding and addressing the unique needs of each patient as you guide them through their rehabilitation journey.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• Possession of an active State License in Physical Therapy is mandatory.
• Basic Life Support (BLS) certification is a prerequisite for employment.
• Strong verbal and written communication skills, with proficiency in documentation and electronic health record management.
• Ability to collaborate effectively with interdisciplinary teams, ensuring a coordinated approach to patient care.

Preferred Experience

• While not mandatory, prior experience in a home care setting or with a similar patient demographic is highly preferred.
• Demonstrated knowledge of orthopedic conditions and rehabilitation practices would be advantageous.
• Experience with patient education and family involvement in treatment plans is beneficial.

Compensation and Benefits

This position offers a competitive annual salary ranging from USD 80,000 to 100,000, commensurate with experience and qualifications.
