Substance abuse counselor

Manage a caseload of no more than 50 patients (40 if not fully certified). Complete screenings, admissions, orient new patients, individual and group counseling no less than once a month, complete all documentation in a timely manner, complete minimum of 3-5 counseling, service plan development, and/or assessment sessions daily, monitor and address patient’s drug screens, complete biopsychosocial assessments, develop individual treatment plans, transitional plans, and discharge plans. Duties also include making appropriate referrals, ensuring annual are completed, keep all patient files organized and updated in accordance with agency’s policies and procedures, monitor patients phases and treatment modality, monitor patient’s attendance, complete phase ups and phase downs based on eligibility, and assist patients with exception and guest dosing requests. Administrative duties and requirements include attendance of staff and treatment team meetings as scheduled, participation in quarterly Peer Chart Reviews, maintain a minimum of 16 hours of annual ongoing education and training, meet and maintain the minimum competencies set for this position, and any other duties as assigned. #J-18808-Ljbffr Gaca
