Responsibilities: Coordinate all intake activities to establish home care for patients. Schedule services as determined by physician orders, contact patient/family, referral source and appropriate staff to coordinate services. Manage the patient schedule to provide timely and coordinated care for all patients. Triage inbound calls from patients, families, physicians and staff members to appropriate clinical or operational personnel ensuring timely, responsive care in the most cost-effective manner. Interface with multidisciplinary team members to provide optimum care. Utilize exceptional communication and interpersonal skills to enhance and encourage collaborative staff and community relationships. Act as liaison to in‑patient case managers and other referral sources in the care coordination of home health services. Receive orders for Home Health services from referral sources. Obtain necessary information to coordinate all services. Coordinate start of service with referral sources and patient. Create and maintain patient & employee schedule to provide timely and coordinated care for all patients. Document coordination in electronic medical record. Serve as hospital liaison to provide in person care coordination for referral management. Meet with patients, case management team & other health care services members to identify home care needs and provide care coordination to ensure a timely discharge and start of services for home health. Develop relationships with referring sources with emphasis on continuity of care within HonorHealth. Provide education to Case Managers on HonorHealth Home Health. Represent HonorHealth Home Health to referring sources. Obtain benefits and eligibility for new patients as well as recertification’s utilizing all resources effectively and efficiently. Other duties as assigned. Education: Associate's Degree – Preferred High School Diploma or GED – Required Experience: 1 year experience in referral, pre‑authorization or scheduling – Required 1 year home health or post‑acute liaison experience – Required
