Responsibilities
- Perform initial physical therapy evaluations and ongoing treatments for residents, with emphasis on mobility, strength, balance, and gait.
- Develop, implement, and modify individualized treatment plans to meet rehabilitation goals.
- Document sessions accurately in the patient records and monitor progress to adjust the care plan as needed.
- Collaborate with nursing staff, therapy assistants, and other members of the interdisciplinary team to maximize functional outcomes and ensure safe, consistent care.
- Educate residents and families on exercise programs, safety strategies, and activities to support continued progress and discharge planning when appropriate.

Requirements
- Active physical therapist license in the United States or eligibility for Wyoming licensure.
- Experience in geriatric or long-term care settings preferred.
- Ability to work 36 hours per week, Days shift.
- Strong communication and documentation skills; ability to collaborate with an interdisciplinary team.
- Comfortable working in a long-term care facility environment and adhering to facility policies and regulatory guidelines.
- Ability to pass credentialing and any required background checks as part of the assignment process.
