This position is fully remote, allowing flexibility while working to support hospice patients and their families. Although the setting is primarily virtual, the LICSW will collaborate closely with a multidisciplinary team of healthcare providers, including physicians, nurses, and support staff. Key Responsibilities\n

• Facilitate communication between patients, family members, and the care team, promoting understanding of treatment options and support services.

• Coordinate crisis interventions, offering immediate emotional and psychological support for patients and their loved ones during challenging times.

• Document all interactions and interventions accurately and in a timely manner to comply with legal requirements and organizational policies.

The primary focus of this role is on patients receiving hospice care, often facing critical health challenges and navigating end-of-life issues.
