Care Coordinator (Please Select) Program: Areta Crowell Wellness & Recovery Center (Downtown SD) Region: Downtown San Diego Services: Adult Outpatient (AOP) Career Type: Counselors Hours : 40 hours per week. Monday, Tuesday, Thursday, Friday, 8:30 am - 5:00 pm or 9:00 am to 5:30 pm; Wednesday, 10:30 am - 7:00 pm or 11:00 am to 7:30 pm. Conduct case management, linkages and referrals to needed supports in the community. Act as a liaison between the clinic and community partners to facilitate referrals and coordination of care for existing clients. Assist the client in meeting client service plan goals, prioritizing areas of need, and steps needed to reach goals established, as well as identified areas of strength. Engage and follow up with disengaged clients.
