Physical Therapist Home Health

Job involves providing physical therapy services in a home health setting. Candidates must have a minimum of 12 months of home health experience within the last three years. Experience with HCHB and OASIS is required. This position guarantees 40 hours of work per week. Candidates must meet a minimum productivity expectation of 32 points per week.
