The Case Manager will assess patient needs, coordinate care, facilitate safe and appropriate discharge, support utilization management, and collaborate with patients, families, providers, and interdisciplinary teams. This position requires a minimum of 2 years of acute-care Case Management/Discharge Planning experience and prior travel experience. Behavioral health-only experience does not meet the requirement. Required Qualifications 2 years of Acute Care Case Management/Discharge Planning experience required. Hospital experience required. Behavioral health experience alone does not qualify. Previous travel nursing experience required. Current RN license required. Pending California license may be accepted at submission but must be active by the start date. Compact RN license must be in hand at submission when applicable. BLS required. Strong experience with acute-care case management and discharge planning. Ability to independently arrange DME, SNF placement, and ambulance transportation. Strong utilization review and care-coordination experience. Core Responsibilities Assess patients' clinical, psychosocial, financial, and discharge needs. Perform comprehensive case management assessments. Coordinate care across the inpatient continuum. Develop and participate in individualized treatment and discharge plans. Facilitate safe, timely, and appropriate discharge planning. Conduct utilization review and evaluate admission/continued-stay criteria. Coordinate DME, SNF placement, ambulance transportation, and other post-acute services. Obtain and coordinate prior authorizations. Work with HMOs, IPAs, CMS requirements, and other healthcare organizations. Organize and participate in family meetings. Collaborate with physicians, nurses, social workers, physical therapists, and other interdisciplinary team members. Support patients and families in navigating healthcare resources. Document all case management activities accurately in the computerized medical record. Promote quality outcomes, patient satisfaction, and cost-effective care. Patient Population The position may serve: Neonates Adolescents Adults Geriatric patients Patient Ratio: Approximately 1:25 days; 1:25 weekends Daily Census: Approximately 25 Interdisciplinary Support: Physical Therapy and Social Services Documentation: Computerized documentation required Orientation: 40 hours Schedule 5x8-hour day shifts 8:00 AM–4:30 PM Weekend coverage required based on department needs. Weekend rotation: approximately every other weekend. No scheduled on-call requirement indicated. Travelers may be expected to float between nearby sister facilities within the scope of their assignment. Travel Requirements First-time travelers are not accepted. Prior travel experience is required. Local radius: 50 miles . Local candidates may be accepted at a lower bill rate according to facility policy. Travelers must be prepared to work according to the contracted schedule and facility needs. Refusal to float to an appropriate nearby sister facility may result in termination and/or loss of guaranteed hours. RTO & Holiday Requirements Maximum preferred RTO: 7 days . RTO must be submitted with the initial application. Post-submission or post-offer RTO may not be considered. Requests exceeding 7 days may require client approval and potentially delay the offer. Traveler must commit to working 3 of 5 holidays : Thanksgiving Christmas Eve Christmas Day New Year's Eve New Year's Day Providence Facility Requirements Only one Providence facility may be submitted to at a time. Candidates must disclose whether they have previously worked for a Providence facility as: Permanent staff Travel staff Registry/per-diem If yes, the candidate must provide the facility location and dates worked. Former Providence staff may return after a 3-month separation . Facility does not accept Nursys or background-check hits. Two professional references are required during compliance, covering at least one year of experience within the last three years. Charge RNs/Leads may be used as references. Compliance Requirements SSN and DOB required for compliance. Compact license must be active/in hand at submission when applicable. California license may pend at submission but must be active before start. Compliance requirements should be initiated promptly after offer acceptance. Modules are completed before start and annually. Modules average approximately 16 hours and are non-billable. More than 20 module hours requires approval. Module completion is documented through Workramp transcripts.
