You will play a key role in helping patients regain independence and achieve personalized therapy goals.Key ResponsibilitiesPerform patient evaluations and develop individualized treatment plansDeliver therapeutic interventions to improve strength, balance, and mobilityMonitor patient progress and adjust treatment plans as neededMaintain accurate and timely documentationCollaborate with interdisciplinary teams to support patient outcomesEducate residents and families on therapy techniques and recovery goalsQualificationsBachelor's degree required; advanced degree preferredActive, unencumbered Physical Therapist license in South CarolinaExperience in a long-term care or rehab setting preferredFamiliarity with PointClickCare (PCC) and Casamba preferredStrong communication and clinical skillsAbility to work independently and as part of a teamWe are an equal opportunity employer.
