Licensed Practical Nurse

Position Summary Licensed Practical Nurse for the Behavioral Health Home program located at 233 Main St New Britain, CT 06051. Responsible for direct oversight and care of Residential and Community Support clients to ensure appropriate medical and psychiatric services. Supervises medication management and reviews referrals to ensure client needs are met. Responsibilities Supervises medication monitoring/administration, writes monthly MAR sheets, trains staff, and participates in weekly team meetings to review client medical and medication issues. Reviews all referrals to Residential and Community Support/clinical programs for medical and psychiatric appropriateness, collaborating with the Medical Director, area directors, and program coordinators to develop treatment plans. Reviews physical health of all clients and provides screenings to ensure compliance with regulations and appropriate triage and referral. Tracks program compliance with JCAHO, DPH, DCF, other licensing and regulatory agencies, and quality‑improvement projects related to client medical status. Provides medical and psychiatric education to clients and their families through individual, group, and family meetings, documenting nursing interventions in the client’s medical record. Performs all other duties as assigned. Requirements Valid DMV license. Licensed Practical Nurse diploma or certificate from an accredited School of Nursing. Bachelor’s degree preferred. LPN licensure in the State of Connecticut. 3–5 years experience in behavioral health nursing. 3–5 years experience in a relevant clinical setting with the population served (children, adult, mental health, and substance abuse). Basic to intermediate experience with Microsoft Office products, including Word and Outlook. Preferred Qualifications Experience with an Electronic Medical Record (EMR) system.
