Position Overview
As a PRN Licensed Medical Social Worker (LMSW) and Volunteer Coordinator, you will:
Complete psychosocial assessments and assist with care planning and counseling for hospice patients and families.
Participate in the interdisciplinary team (IDG/IDT) to support the patient’s overall plan of care.
Provide guidance and support with advanced directives, community resources, and emotional support.
Recruit, train, and supervise hospice volunteers, ensuring compliance with federal and state volunteer hour requirements (minimum 5% of total care hours).
Maintain required volunteer documentation, reports, and compliance records.
